Faculty must submit a Consultant Agreement Form (Only if payment is over $5000).

Foreign National Form must be completed along with ayour I-94 Form “Arrival and Depassport), copy of your U.S. VISA from your passport, and 1-20 or IAP66 must be attached to this form.
Copy of Social Security Cards or ITIN#.
